Frequently Asked Questions about Registration at a CCSD school

What documents do I need to take with me when I register?
• Personal identification of parent/guardian (driver's license, picture identification, etc.) bearing your name.
• Proof of the child's identity, such as the original birth certificate, a passport or a certified birth card issued by a health district. Photocopies not accepted. This is for students who are coming from out of state.
• Up-to-date medical records indicating that your child has had, or at least started, the following series of immunizations:
-Four DPT shots (but one more if last dose was given before 4th birthday).
-Three polio doses (but one more if last dose was given before 4th birthday).
-Two MMR shots (one dose must be given on or after the 1st birthday).

What if my child has never attended a school in Nevada?
• Two Hepatitis A shots (2nd dose must be given at least 6 months after the 1st dose).
• Three Hepatitis B shots (must have a minimum of 4 months between 1st and 3rd dose).
• One (1) Chicken pox (Varicella) shot (must be given at 12 months of age or older). Physician verification required if student has had the chicken pox disease.

Note: A child enrolling in seventh grade after June 30, 2008 is required to have the Bordetella Pertussis vaccine ("Tdap"). The child must receive at least one dose of a Bordetella pertussis-containing vaccine after age 10.

Is there an age waiver for students from other states?
A child who becomes a resident of this State after completing kindergarten or beginning first grade in another state in accordance with the laws of that state may be admitted to the grade he was attending or would be attending had he remained a resident of the other state regardless of his age, unless the board of trustees of the school district determines that the requirements of this section are being deliberately circumvented. (NRS 392.040(8))

Where do I go to register my child?
Register at the school your child is zoned to attend. You will need the following items:

• 4 Proof of address. One item proving your address, such as a recent utility bill, rent receipt, residential lease or sales contract.

Note: The following items CANNOT be used as proof of address:
• driver's license, telephone bill or cable bill.

If you are living with a friend or relative due to economic hardship, you must bring:

• A notarized statement signed by you and the owner/renter of the property indicating that it is your place of residence. This form is available from the Registrar here at Canyon Springs.

• If you do not have the above documentation, please call the Homeless Outreach Program Office at 855-6682.

How can I make sure my child is placed in the appropriate class or program?

To make sure your child is placed in the appropriate class or program, please provide the following information:
• Previous records regarding placement in a special program or class.
• A recent report card or transfer document showing the name and address of your child's last school.
• Information about any disabilities or special health problems, such as seizures, asthma, heart problems, health care procedures or medications.
Please contact the school nurse if your child has health concerns which may impact him/her at school.

My child has been attending public school in Clark County, but we are moving to another school within the district. How do I transfer my child?

Go to your current school and withdraw your child. Take the transfer papers to the new school. Also, take proof of your new address (see proof of address requirements above).
