Enrollment Information
If your child is enrolling in a Clark County school for the
first time, or if you are re-enrolling your child in a Clark County
school, you will need the following items to complete your child's
registration:
1. Proof of
address.
Two forms
of proof of address are required, such as a recent utility bill,
rent receipt, residence lease, or sales contract. If you only have a
sales contract for the prurchase of a new home as proof of address,
the school will enroll your child on the condition that one
additional proof of address from the above list is submitted within
30 days. (Driver's license, telephone bill, or cable bill is
NOT
acceptable.)
If you are
living with a friend or relative, you must bring:
-
a
notarized statement signed by you and the owner/renter of the
property where you are staying, indicating that it is your place of
residence.
-
a form of
your personal identification (driver's license, picture I.D., etc.)
which bears the address of the friend or relative you reside with.
(Must be presented within 30 days.)
2. Proof of
the child's identity.
This may
consist of an original birth certificate, a passport, or a certified
birth
card (issued by the Clark County Health district or another
health district). Copies will
not be accepted.
3. All parents registering their child must provide a picture ID.
