General Registration Information:

If your address is an EVEN Number your child will attend McMillan Elementary

If your address is an ODD Number your child will attend Katz Elementary

(For example - if you live in an apartment at 1315 Lake Mead, in building 27, apt 100 you need to register at McMillan Elem. The apartment address does not count the building number does not count, the apt # counts. If you live in a house at 1315 Lake Mead, you will need to register at Katz Elem.)

Requirements for kindergarten enrollment:

Child must be five years of age on or before September 30, 2007

Must have:

Original Birth certificate. (copies can not be accepted)

Immunization record

Two Proofs of Address. CCSD accepts - mortgage, lease, power,gas,and water bills as proof or address. OR

Notarized letter of homeowner you reside with, Including their proofs of address.

Please complete all forms and ensure you have SIGNED THEM.
