Enrollment and Withdrawal Information

 

When a student enrolls, s/he will not attend class until the next school day.  If you plan to withdraw your child, the office must have at least one day’s notice in order to prepare records.

 

 

REGISTRATION INFORMATION

Proof of address

You must bring two of the following:

  • Recent utility bill
  • Rent receipt
  • Residence Lease or Sales Contract

Note: If you only have a sales contract for the purchase of a new home as proof of address, the school will enroll your child on the condition that one additional proof of address from the above list is submitted within 30 days. Driver's license, telephone bill or cable bill are NOT acceptable.

If you are living with a friend or relative you must bring:

  • Notarized statement signed by you and the owner/renter of the property where you are staying, indicating that it is your place of residence.
  • Form of your personal identification (driver's license, picture I.D., etc.) which bears the address of the friend or relative with whom you reside. (Must be presented within 30 days of enrollment.)

If you do not have the above documentation because you are living in a temporary or homeless situation please call Title I Homeless Outreach Program for Education (HOPE) at 855-6682.


Proof of child's identity

Please bring ONE of the following:
(Photocopies will not be accepted.)

  • Original birth certificate
  • Passport

Certified birth card (issued by the Clark County Health District or another health district.


Proof of immunizations

You must have records indicating that your child has had, or at least started, this series of immunizations:

  • Four (4) DPT Shots (Add one more if last dose was given before fourth birthday.)
  • Three (3) Polio Doses (Add one more if last dose was given before fourth birthday.)
  • Two (2) MMR Shots (one dose must be given on or after the first birthday. A second dose is required for entry into kindergarten and first grade.)

For students new to Nevada schools (whether entering school for the first time or transferring from another state):

  • Two Hepatitis A Shots (2nd dose must be given at least 6 months after the 1st dose.)
  • Three Hepatitis B Shots (Must have a minimum of 4 months between 1st and 3rd dose.)

Chickenpox (Varicella) shots (2nd shot only required is 1st shot given after the age of 13 years. Minimum of two months spacing between shots required.)
