Registration Checklist
For a transfer within CCSD:
1. 2 Proofs of address:
Note: Proofs of address
must be in parents
or guardian’s name
*Power, water or gas bill
*And/ or rental agreement or
*Mortage statement
“If you are currently residing wtih a friend or
relative,
a notarized statement signed by the residence is
required, includeding two proofs in their name.
Additionally, your
photo ID must bear the address of the friend or
relative”
2. Parent or guardian photo ID
3. Withdrawal form from previous school
(if application, only required during the regular school
year)
4. Completed and signed registration forms
*Registration form CCF 705
*Blue health card CCF 768
*Elective form
Students new to CCSD:
1. Above documentation required for transfer within CCSD
2. Original Certified Birth
Certificate
3. Immunization Records
