Required Registration Information

Registration must be done in person by a parent or legal guardian. You must have a photo ID with you. The following documents are required at the time of registration:

1. Official State Birth Certificate (hospital certificates will not be accepted).
2. Immunization Record.
3. Proof of address: We accept Utility bills (water, gas, sewage), Lease agreement, Mortgage payment. No phone or cable bills!

These items are required prior to receiving the enrollment forms.

Registration papers will not be given out until the requested information is received.
