FALL REGISTRATION

Registration for students new to Elise L. Wolff Elementary for the 2009-2010 school year will begin June 8, 2009 and continue through the months of
June, July and August.
Registration hours will be Monday - Friday 8:00 a.m. - 11:00 a.m.

Items needed for registration:

• An original Birth Certificate
• One Proof of Address

 (a recent bill from Southwest Gas, Nevada Energy, Henderson Utility,
Las Vegas Water, Rent/Mortgage receipt or sales contract)

• Picture ID of parent or guardian
• Up-to-date immunizations

Five (5) DPT shots
Four (4) Polio Doses
Two (2) MMR shots (1st dose given on or after the first birthday)
Hepatitis A series (two doses 6 months apart. Can be in progress)
Hepatitis B series (three doses)
Varicella – (The Chicken Pox vaccine – One shot required for all grades. 
First shot must be at 12 months of age or older.)
History of chicken pox disease will ONLY be accepted by a physician’s note.

 

Track consideration is applied on a case-by-case basis.

Kindergarten Students:
5 years of age by September 30, 2009
